Persons Served

Individuals with the following disabilities are served:
  • Mental Retardation

  • Downs Syndrome

  • Cerebral Palsy

  • Brain Injury

  • Seizure Disorder

  • Prader Willi Syndrome

  • Autism

  • Tuberous Sclerosis

  • Other disabilities

Admission Procedure

  1. Family initially contacts the Director of Admissions (712-324-4873) about interest in program.

  2. Individual & family are encouraged to tour Village Northwest and receive information about program. We also encourage families to visit other programs in order to compare services.

  3. If individual and family are interested in program, background material (school, vocational, medical records) is obtained. Individual is encouraged to come to the Village for pre-admission evaluation. Also, they may meet staff members who would be working with the individual.

  4. All information is evaluated to determine if the Village can meet the individual’s needs. If so, application packet is given to family.

  5. Family/individual complete application packet.

  6. Family/individual contact County Department of Human Services to explore funding for program.

  7. Application is evaluated by admissions team and is accepted/rejected.

  8. If accepted, application is kept "open file" until an appropriate opening occurs.

  9. When an opening occurs, all applicants are considered.

Initial admission is for 30 days, at which time a formal admission staffing is held.

Admission Criteria
MR/DD Services

  • Individuals will be considered for admission, regardless of race, religion, sex, or national origin.

  • Must be at least 17 years of age.

  • Must have a developmental disability that requires support in residential and/or vocational settings.

  • Must have a source of funding to pay for necessary services.

  • Health must not prevent individual from actively participating in services.

  • Must not be of danger to self or others.

  • If guardian is deemed necessary by the Interdisciplinary Team, including the family members, appointment should be made within one year.

  • Individual and/or guardian must agree to contracting for services from Village Northwest.

  • Must be free of contagious diseases.

  • Must be medically stable and not require 24-hr skilled nursing care.

Admission Criteria
Brain Injury Services

  • Preference will be given to individuals who are the most recent post-injured and who are exhibiting the greatest potential for growth.

  • Must be at least 17 years of age.

  • Must demonstrate a level of endurance, which would allow him/her to engage in an individualized program.

  • Must be free of current drug or alcohol dependency.

  • Must be free of psychosis which would hinder his/ her ability to participate in an individualized daily schedule.

  • Must be free of behaviors that pose a threat to the safety and well being of self, as well as others.

  • If guardian is deemed necessary by the Interdisciplinary Team, including the family members, process should be implemented immediately.

  • Must have a funding source to pay for necessary services.

  • Must be free of contagious diseases.

  • Must be medically stable and not require 24-hour skilled nursing care.
